Attendance Policy and Payment
* You may request a free 15 minute meet and greet telephone or video session to ensure we are a good fit.
Payment
Payment is due at the beginning of each session; unless otherwise negotiated. I currently only accept credit/debit cards or HSA cards for payment through Simple Practice (a HIPAA-compliant Electronic Health Record software program). In-network insurance billing will be completed electronically through Simple Practice following the session.
Phone Calls
I do not charge for phone calls under 15 minutes. However, if that call should exceed 15 minutes with clients or collateral contacts (i.e. school counselors, teachers, doctors, etc.) a prorated fee will be charged. Please note that many insurance companies will not reimburse for phone contact.
Insurance
I am currently in network with Premera, Blue Cross, Lifewise, and First Choice Health Network. For other insurance providers, I am considered to be an "out-of-network" provider. Please check with your insurance company to determine your out-of-network benefits. I can provide you with a superbill to submit to your insurance provider for reimbursement. It is likely your insurance company will cover a portion of the cost. If you choose to bill insurance, a mental health diagnosis is required, which would be identified in the Initial Intake Session.
When speaking with your insurance provider, you may consider asking the following:
*Do I need prior approval/referral from my primary care physician?
*How many outpatient mental health sessions are covered per year?
*Is family therapy covered? If so, is a mental health diagnosis for one member required for reimbursement?
*What is my deductible and has is been met?
*What is my reimbursement rate for an out-of-network provider?
*Where can I find the form necessary to submit my own medical claim for reimbursement?
Sliding Scale
As best I can, I try to reduce any financial barriers when it comes to receiving services. Reduced fee sessions are subject to availability.
No Surprises Billing Act
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost under the law, health care providers need to give patients who don’t have insurance or who are not using insurance, an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
Cancellations
Please provide 24 hours notice if you need to cancel or reschedule an appointment and will not be charged a cancellation fee. Late cancellations (within 23-0 hrs of scheduled session) will be charged half of the scheduled session fee. If there are more than two cancellations and/or No Shows in a month and prior arrangements have not been made, your services may be suspended and your appointment time released.
Late Window
If you are more than 15 minutes late for a therapy session, the session will be canceled and considered a No Show.
If you communicate potential tardiness in advance, the decision to conduct the session will be up to the therapist.
No Call/No Show Fee
The full scheduled session fee will be charged without advanced notice of cancellation.
